FINAL IAEA Draft, In late January 1978 an International Atomic Energy Agency (IAEA) nuclear safeguards inspector, Pierre Noir, died while inspecting a nuclear reactor in Taiwan. Initial reports indicated Noir had died of a heart attack. However, after carefully examining the TV surveillance equipment that Noir was working on I proved he had actually been electrocuted because of an electrical wiring flaw. This memoir, which I wrote in 2022, details my investigation.
The death of inspector Noir has had limited discussions in the open literature. Here is what I’ve found:
